Records Request

UIHS values your privacy and keeps your health records secure and confidential. There may be times when you need to have access to your records. UIHS provides several ways that you can request your health records:

• In person. Stop into any clinic to request your health records and our friendly staff will assist you.
• Through our patient portal.
• Electronic requests direct from our partner, HealthMark Group. Requests may be submitted electronically to HealthMark’s Request Manager. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed and a notification will be sent via mail or email once complete and available for download.
