Community Health Care
The Community Health Care Sections offers a wide range of services that include:
Head Start: Collaboration with local tribal Head Start programs to provide vision and hearing screenings to children ages 3 – 5 years.
Blood Pressure and Blood Sugar Screenings: Offered during home visits, and at nutrition sites throughout both counties every month.
Injury Prevention: Community Health Representatives (CHR’s) provide education and resources to our communities through our Injury Prevention program. This program provides car seats(Donation requested), bike helmets and smoke detectors for all of our communities.
Family Spirit Program: The CHR’s offer a home visiting program which provides education and resources to pregnant clients and families with young children. These weekly visits with families are done in individual and group settings. This program provides prenatal education through early childhood development up to age 3 years.
Preventative Health: The CHRs offer community services such as head checks at tribal summer camps, ASQs (developmental assessments for children), nutrition information and assistance with connecting to resources (i.e. WIC).
Immunization Clinics: The CHRs may provide flu vaccines as well as other immunizations at various events in the communities as well as at UIHS clinic sites.
Elder Services: The CHRs are able to provide home visits to Elders in all of our communities. These home visits can provide anything from just a check-in visit to delivery of medications, education on disease process or blood sugar or blood pressure screenings. We also can offer nursing services with home visits as needed.
Referrals: The CHRs receive and respond to referrals generated by the providers, nurses, community members and family members.
Transportation services to appointments at Potawot Health Village are available Monday, Tuesday, Thursday and Friday mornings each week. If you need help with getting to your appointments please let the person scheduling your visit know at the time you schedule. This will help avoid missed appointments or confusion.
